Medical Emergency Card

During these unprecedented challenges, our main concern is your family’s safety and well-being. It is possible that your child or family member may be seen by a doctor or healthcare provider who is not familiar with them, their medical history, or their genetic condition.

For this reason, we encourage you to create a medical emergency index card for you or your child(ren) that lists their main health issues. Be sure to include general information about their condition that may be relevant for their care should they need medical attention. To assist you with this project, we have created a template that you can use for your child’s information.

5. Keep in your bag or wallet in case of a hospital visit
